You can fit a lot of information in 5 lines by abbreviating. For example one line of my tag says:
NKA A+ Organ Donor
Hopefully it's pretty obvious to an EMT or someone that it means No Known Allergies, Blood type is A+ and that I'm an Organ Donor.
Another line says
ICE:9991234567
And yup.. I wear 'em alla the time.

It can be debated whether it's easier to forget your dog tags or your helmet on a ride, probably wise to do what works best for you.
In the 70's and 80's I was a volunteer fireman/EMT for a small rural fire department. We had an ambulance and on a run once to respond to a MVA I was taking vitals on a semi-conscious victim. One of the firemen who was on the scene ( a logger by the way) was recording the info. I gave the vitals; bp pulse, pupil observation, breathing etc. I also stated that the victim was a diabetic and wore contact lenses, I also told him the victims emergency contact telephone number. The logger stopped me there and asked incredulously how I could know these things from my rudimentary assessment. He was pretty impressed with my keen medical skills. The victim wore a wrist ID bracelet and it was right there as I took the wrist pulse.
A helmet might be a better location for some people but the reality is you are well served to have this info on you at all times. Though you may be at a higher risk than normal when on a bike, think about what percentage of your life you spend on a bike (not counting Kent Peterson here) compared to the rest of the time. As a past EMT I can tell you easy access to such info was not common but greatly appreciated when it was available.

About 2 years ago I got myself the "Dog Tag" style of Road Id, called "Fixx
ID". What I really like about it is that I always have it with me. I don't
"forget it" when I leave home without my wallet or because I forgot to put
something on. I wear it night and day, in the shower, always, and I hardly
notice it. I'm rather practical, so purchased the stainless steel, but if
you've money to burn, you can get your id in 10k gold but you supply the
chain.
I encourage every cyclist to wear something like this.
